Jennifer Lui, MSW, LCSW
Jennifer is the Regional Director of Population Health Clinical Services with the University of North Carolina Health Alliance. She Is an experienced healthcare leader with a demonstrated history of working in the ever-changing health care environment. She leverages her deep understanding of medical, behavioral and social needs to facilitate value-based Population Health strategies. She is skilled in change implementation, measurement, evaluation as well as in human resource management.
Prior to joining UNC Health Care, Jennifer was a Principle Program Manager with Blue Shield of California. She led the development and implementation of Palliative Care program for the Accountable Health Organization (ACO) contracted health system throughout the State of California. In addition, Jennifer spent six years at Providence St Joseph Health, where she managed the clinical ambulatory Care Management department. She developed and led implementation of several high-value initiatives, including a proactive outreach Case Management program and the expansion of the ambulatory Care Management department. On top of that, she led the system-wide Palliative Care steering committees, and in charge of the POLST and ePOLST interoperability efforts.
Jennifer holds a Master’s degree in Social Work from the School of Social Work at the Portland State University, a Bachelor of Science degree in Psychology at the Portland State University, a certificate in LEAN, a certififate in Change Acceleration Process, and is licensed as a Clinical Social Worker in the states of California, North Carolina, Oregon and Washington.
