Ellen Aliberti, MS, BSN, PHN,CCM, RN
Ellen Aliberti, RN, BSN, CCM, MS is a seasoned managed care leader that has over 30 years of case management experience within in managed care organizations. Ellen has worked in most settings in various leadership roles including Director of Acute Care Management and Director of Post-Acute Care Services. She is a certified case manager and currently is the Lead Clinical Educator for HealthCare Partners. She has an extensive background in managed care, case management, and clinical process improvement and has led many interdisciplinary teams focused on developing programs and pathways directed at enhancing the effectiveness and quality of patient care transitions. Ellen is a prolific writer and national speaker on care management and managed care and is currently writing for an online CEU/CCM provider. She is the founding president of the Case Management Association of Las Vegas and an Advisory Board member of the Practice Change Fellows a national organization focused on mentoring new leaders in geriatric care (2003-2010); Advisory Board member for the RWJ Foundation sponsored HMO workgroup on Care Management (1994-2003); recipient of the So. NV Chapter March of Dimes Nurse of the Year in Managed Care (2006) and recipient of the So. NV Chapter of the March of Dimes Nurse of the Year in Education (2011). In 2017, Ellen was also honored as the Case Manager of the Year by the Case Management Society of America (CMSA). She also is a licensed Life and Health Insurance producer in Nevada focused on Long Term Care (LTC) products.
