SY02 – Integrated Care Management as Key Population Health Management Strategy
Centene Corporation’s approach to care management and population health focuses on providing integrated, holistic plans to ensure our members receive the support and services they choose. This symposium will illustrate the use of data science and machine learning predictive models in various phases of the care management process: the member’s identification, risk stratification, engagement, and outcomes measurement. In addition, a review of how our members’ needs are assessed and how our members and their chosen care team direct the goals that are developed, as well as the different types and levels of care management support that are available. Outreach methods utilized, both virtually and in-person, as well as how we developed the model of community health workers to address SDoH barriers will be examined, in addition to an exploration of the relationships between the member, data, the person centered team and the health outcomes for members. Member success stories will be shared, supported by videos and care strategies that are developed and reflected in each person centered plan. We will discuss how we have modeled our approach around CMSA standards of practice and continue to encourage and support all Care Management team members to possess or obtain CCM and/or Health Coaching and Person Centered Thinking certification as foundational pieces that contribute to the success of the whole person and subsequently the population health.
