Susan Fegen, LVN, PCT
Susan has 15 years of experience in Managed Care, with an emphasis on the elderly, chronic conditions and foster care. She has been a nurse for over 20 years, and is a Person Centered Mentor Trainer with The Learning Community for Person Centered Practices. Currently Susan is the Manager of Person Centered Programs for Centene Corporation. She has been involved in the development of a digital Person Centered assessment tool, Person Centered Brochures and videos to bring PCT information to persons with low vision or reading deficits.
Susan currently sits on the National Qualify Forum’s Person Centered Planning Committee. This committee is actively providing input on a measurement framework for person-centered planning; which will include a set of core competencies, a refined definition for PCP, and recommendations for systemic characteristics that support person centered thinking, planning and practices.
