I have had the privilege of working within the field of nursing for over twenty years. My healthcare career has focused on the care and recovery of high risk patients. I began as a registered nurse in a Cardiothoracic ICU caring for critically ill post- operative patients. It was during this time that I was exposed to the unique and integral role that Nurse Practitioners play in ensuring the health and well-being of their patients while improving outcomes. Their invaluable, all-encompassing, holistic care is what inspired me to acquire my ANP degree. My current work continues to focus on high risk patients with the emphasis now on the transitional care process. As a Care Navigator, I am ensuring that the medical and psychosocial needs of patients are being met upon discharge home and within the community setting. The experience of working collaboratively with inpatient and community providers to “care for those who need it the most” as our motto states, has been an amazing experience.
