Anita works as a Practice Transformation Coach for Community Care of North Carolina, where she provides innovative tools and support to help independent community practices continue to deliver high-quality, value-based care in an efficient practice setting to ensure viability under Medicare & Medicaid reform. Anita has over 10 years of Case Management experience and worked as a Care Management Support Coordinator. Other roles have included Quality Improvement Core Team member and QI Lead for the NC Central Carolina Region that provided technical assistance/support to other QI Teams and community practices. She is a trained Lay Leader for Chronic Disease Self-Management, and has received additional training with Charlotte AHEC and the Institute for HealthCare Improvement. Anita is a former graduate of the University of North Carolina at Chapel Hill.
