PP01 – Innovative Strategies in Case Management Education, Working as Part of an Interdisciplinary Team
Northwell Health Solutions division encompasses multiple innovative care management and coordination programs such as transitional care, health home, disease management, advanced illness, and call center. The company employs nurses, nurse practitioners, social workers, physicians, and non-clinical personnel. Many hired employees are unfamiliar with updated standards of care, latest government regulations, core measures, and documentation requirements in care coordination of complex population. There is also insufficient knowledge of developments in the disease specific care management strategies and ways in patient engagement.
In order to ensure high quality care management for all patients, the staff requires initial and ongoing education. Many of the important concepts include patient-centered care, health literacy, cultural competence, care management and coordination, and interdisciplinary collaboration. Innovative and interactive educational strategies are necessary to ensure staff’s sufficient knowledge and satisfactory competencies. Education department developed a strategic plan and tools to increase development, use and evaluation of innovative education approaches for multidisciplinary care managers and coordinators. The poster presentation outlines the strategic education plan, includes visual images of the tools that have been created, and shows expected outcomes. Innovative methods proposed and discussed are simulation, videos, self-learning interactive modules, and case scenarios. Expected outcomes include improved staff satisfaction and better patient satisfaction with care coordination provided.
