4008 – Continuum of Care-Positive Outcomes within Workers’ Compensation

Combining the early application and persistent delivery of case management services across the continuum of care has resulted in positive outcomes demonstrated within the workers’ compensation industry.  While measurements of the patient’s level of function are determined by the medical team, the results of effective case management are often seen in the patient’s ability to remain at work or return to work.  This unique interplay of the medical status and the employment status within the workers compensation environment have provided the opportunity to measure the positive effects of case management interventions.  Additionally, case management within the federal population enables analytics not influenced by claim settlements as seen in the private sector.
This presentation will provide analytics that demonstrate the effective delivery of case management services resulting in the reduction of lost days of employment and reduction of compensation costs.  Analysis also offers examples of a positive return-on-investment for the employer utilizing case management services. 
The presenters will share descriptions of techniques and sample cases to demonstrate the cause and effect and link between the case management technique and outcome with demonstration of the application of the Facilitation, Coordination and Collaboration Standard of Practice.   As an interactive session, participants will be asked to project expectations of outcomes with comparison to actual results. Discussion will include designs within the case management plan with utilization of reports and tips for analysis of reports to avoid missed aspects of the plan.  Application of written and verbal communication techniques will be explored to enhance both the case manager and supervisor follow-up with various members of the interdisciplinary team in working toward positive resolution and sharing of outcome measurements.

Objectives:
1) Participants will recognize and identify outcome measurements that demonstrate effective case management across the continuum of care with application of the Outcomes Standard of Practice

2) With enhanced understanding of the Facilitation, Coordination and Collaboration Standard of Practice, participants will be able to identify and describe the effective case management techniques that influenced the positive outcomes

3) Participants will apply techniques of facilitation, coordination and collaboration resulting in positive outcomes
